生醫西進 兩岸互補吃大餅 2012-06-04 00:55 工商時報 【記者杜蕙蓉/專訪】 大陸國務院總理溫家寶530定調,加速發展節能環保、新一代信息技術、生物、高端裝備製造、新能源、新材料和新能源汽車等七大新興產業,其中,「生物醫藥」擴大為「生物」,延伸至包括生物工程、生物製藥、生物農業、生物能源等多個領域。  Vivo創投創辦人孔繁建認為,在大陸已成為全球第三大醫藥市場下,生醫廠商應把大陸列為卡位的目標市場;當年針對生醫廠商的發展方向是要以符合美國FDA或大陸SFDA認證標準時,現在的製程規格可以考慮以符合SFDA為基礎,鎖定檢測、醫材、新劑型和華人特有疾病新藥領域 孔繁建表示,不要怕兩岸醫藥開放,台灣市場太小,陸資藥廠不會想來賣藥,而是想找技術、臨床和台灣對國際專利法規藥廠的認識,這是雙方合作利基。以下為專訪紀要:  

問:Vivo今年完成規模高達3.75億美元的生技基金募集,為何將大陸的投資比重由過去的20%提高為50%?  答:大陸已成全球僅次於美國和日本的第三大醫藥市場,預估其市場將由2010年的410億美元,以每年平均22%的成長力道,預計2015年即可達到1,250億美元,因此,國內生醫廠商一定要盡早介入布局。  

問:國內廠商利基為何?  答:兩岸現在醫藥合作協定開放遲遲無法落實,將阻礙生醫廠商進軍大陸,實際上台灣市場太小,陸資藥廠對賣藥到台灣不會有興趣,他們相中的是台灣包括生產、臨床等技術,且國內藥廠對尋求國際法規的認證也較有概念,而大陸市場則有龐大商機和資金優勢。  

問:那些領域較具利基?  答:台灣的檢測、新劑型技術都比大陸藥廠強,像檢測診斷技術,大陸約落後台灣5~10年,在檢測診斷結合藥品開發已成生醫趨勢下,新藥開發風險太高,廠商可投入檢測診斷發展,而藥物傳輸即新劑型開發,台灣也有很強的競爭機會。

潘懷宗..北市醫療糾紛日益頻繁!市民申請衛生局調處一年600件!




2012/06/01 政府應協助加強民眾醫療常識及醫病溝通,以降低醫療糾紛。潘懷宗議員近日接獲大量市民陳情各類醫療糾紛的案件,目前已請衛生局協助安排醫療爭議調處,但衛生局醫療爭議調處安排協調會的時間程序需費時一至二個月以上的時間,潘懷宗議員先將4件有關超音波抽脂及隆胸的案例提出討論。因為擔心即將進入暑假及畢業潮,許多年輕學子會想利用暑假較長的時間進行整形手術,或部分畢業生有時想讓自己的外在條件更佳而讓求職順利,也會求助醫美醫療院所。潘懷宗議員也希望藉由本次記者會呼籲所有愛美的民眾,在進行手術前必須要注意哪些事項,不要讓自己一時的衝動反而造成一輩子的遺憾。潘懷宗議員接獲這4件醫美醫療糾紛的陳情案,1件有麻醉過量疑慮,1件則有麻醉量不足疑慮,4個案例共通點都是進行體外超音波抽脂手術,術後陳情人都認為造成抽脂部位嚴重凹凸不平及遺留明顯手術疤痕,甚至認為診所聘請非合格護理人員協助醫療行為等問題。

案例詳細說明:

案例一、A女在某知名醫美診所進行超音波抽脂手術,花費12萬。()該醫師不看診問診,直接安排進行手術。()手術麻醉非醫師本人,看到醫師沒過多久就昏過去了,手術現場並未有專業麻醉醫師或護理人員告知要開始進行麻醉。造成手術後醒來產生大量幻覺、胡言亂語、咬牙晃頭的情況,還被診所人員認為有精神病史,而且在麻醉未完全退的情況下,因為診所要打烊,被強迫要求離開診所,接下來約半個月都呈現大舌頭且無法控制說話清楚,甚至記憶力衰退的情形。()術後腿部凹凸不平非常嚴重,醫師說等半年恢復看看,過了半年醫生又說再等一年看看,A女認為醫師只是想無限期的拖延,根本不想面對問題負責。()術後一年,手術部位凹凸不平沒有復原的跡象,醫師也口頭承認手術失敗,A女要求退回手術費用及賠償日後進行修復手術的賠償金,竟被還被該醫師報案控告恐嚇。()A女質疑當日在手術房內的兩位著護士服的協助人員非合格護理人員,目前已請衛生局協助調查。()目前該案正在衛生局進行相關協調當中。

案例二、B女在某知名醫美診所進行超音波抽脂手術,花費12萬。()手術過程雖然事先有進行麻醉,但是全程都能感覺到非常疼痛,B女有呼喊但是醫師沒有在手術過程當中針對B女的疼痛進行任何相關協助。()手術已完成1個多月,腿完全無法蹲下,只能使用坐式馬桶,完全無法使用蹲式馬桶。()該醫師在高雄市也有開診所,遂以北部沒有排診為由,無法讓B女回診瞭解術後情形,B女到別間診所諮詢後發現自己腿部原本要抽脂的部分並沒有抽完全,屁股以下的微笑線根本沒有抽。()B女質疑當日在手術房內的穿著護士服的協助人員非合格護理人員,目前已請衛生局協助調查。()目前該案也在衛生局進行相關協調當中。

案例三、C女在某知名醫美診所進行超音波抽脂手術,花費10萬。()術後已經1年,腿部依然凹凸不平無法復原,而且原本希望抽脂的部分也沒有抽完全。()C女到別間診所諮詢,發現原本抽脂的馬鞍部分沒有抽完全,大腿內側也造成嚴重凹凸不平。()D女質疑該診所穿著護士服的協助醫師進行手術者非合格護理人員,目前已請衛生局協助調查。()目前該案也在衛生局進行相關協調當中。

案例四、D女在某知名醫美診所進行超音波抽脂手術,花費30萬。()在該診所進行手臂、肚子、馬鞍、大腿內外側抽脂及自體脂肪墊胸手術()術後胸部出現多處硬塊,到另一家大型醫院進行乳房攝影及超音波,檢查發現許多大大小小的脂肪顆粒。()手臂、腹部術後並無明顯差別,大腿內側更是凹凸不平、某些區塊更是下陷嚴重。()傷口拆線時非醫師而為護士,D女質疑該名護士非合格護理人員,且醫師後來也發現傷口根本還不能拆線,造成現在疤痕非常明顯與難看。()目前該案也在衛生局進行相關協調當中。潘懷宗議員不希望出現更多的醫療糾紛案件,因此先請台北榮總麻醉部鄒樂起主治醫師,針對上述案例中有關麻醉部分做簡易說明及民眾進行麻醉相關手術時,需要注意的事項:. A女手術後產生幻覺、胡言亂語的類似精神錯亂症狀稱為「手術後譫妄」,此情形常見於手術後麻醉甦醒的期間,常見於術後恢復室或加護病房,多半可以由有經驗的醫護人員處理下來。譫妄(delirium)為一種急性的意識障礙或認知改變。特徵主要為意識清醒度下降、注意力變差、失去定向感、情緒激動或呆滯、睡眠-清醒週期混亂、有時清醒有時昏睡,常常伴隨著妄想(例如相信有人要加害於他)及幻覺(常見為影像幻覺或聽到別人指責或指令),因而產生燥動或攻擊行為。術後發生譫妄的誘因有很多種,最常見的為老年、失智症、重大手術(尤其心臟手術及髖關節手術)、術後疼痛、貧血、術前使用藥物(類嗎啡止痛藥、鎮靜劑、安眠藥、抗膽鹼性藥物)、缺水、電解質、不當的肢體約束、未發覺病人急需如廁等等。病患本身併存之疾病如中樞神經病變、甲狀腺疾病、失智症等,或是任何可引起代謝性腦病變的疾病,較常見疾病如全身性感染或發燒、敗血症、癲癇、中風、腦損傷及長時間手術等皆可能導致譫妄發生。A女術後發生譫妄的誘因較可能是貧血、術後止痛不當或精神壓力大等因素。. B女遇到的問題為手術全程感覺疼痛,合宜的麻醉應包括安眠及足夠的止痛,在手術過程中醫師應按步就序給予有效止痛(針劑或肌肉注射),也可添加局部麻藥,以減少術後疼痛的發生。. 一般情況下,當病人經歷靜脈式全身麻醉(上述二案例較可能為靜脈式全身麻醉)或一般全身麻醉後,應有相熟可信賴之親友陪同,確保回家的路程安全。當天不宜騎車、開車、操作重機械器具或簽署重要合約等,術後容易因判斷力不佳而造成意外。而且醫療院所應該負責確認病人的心跳、血壓、呼吸及神智恢復到正常狀況,才能讓病人離院。麻醉醫學會目前亦要求合格的麻醉專科醫師必須達到手術全程監控及完全疼痛控制的程度。

關於病人於麻醉後恢復室的安全注意事項:一般恢復室的設計就是緊鄰手術室,如果病人需要外科處理,就可以就近送回手術室。多半會設計成開放式的空間,以方便醫護人員觀察病人,每位病人的身邊都有監視器。從麻醉中恢復過來對病人生理上而言,是一個很大的壓力。所以在送到恢復室後,很多情形都有可能發生,常發生的問題包括:疼痛、血壓過低、心悸、激動、血壓過高、噁心嘔吐、寒顫、發紺、換氣不足、喉部痙攣、體溫過低、不易叫醒等等,每種問題醫護人員都會立即給予最妥善的處理。因此所有接受全身麻醉及傳導麻醉患者,都必須接入恢復室觀察。以麻醉恢復室作業安全指引建議,病人接入後,前30分鐘需每5分鐘測量血壓、心跳及呼吸次數一次,情況穩定後可每10-15分鐘測量一次,另心電圖及血氧濃度則需全程監測直至病患離開恢復室為止。病人在麻醉甦醒期間,應有安靜的環境讓體內的麻醉藥物減退代謝掉,時間約1-2小時,其間應有醫護人員監測病人的心率、血壓、呼吸次數、血中氧氣飽和度、意識清醒度、活動能力等指標。待病人(a)意識完全清醒、(b)四肢可自由活動、(c)血壓恢復術前血壓八成以上、(d)呼吸狀態為聽從指示後可作咳嗽及深呼吸、(e)皮膚顏色為正常的粉紅色等五個指標恢復正常,才可讓病人離開。門診手術麻醉之病人出院為(1)生命徵象穩定、意識清醒、走路平穩; (2)沒有明顯的噁心、嘔吐或疼痛; (3)沒有出血或明顯的外科手術之併發症; (4)需由可信賴的家屬或朋友陪同回家靜養,休息。回家後第一次喝水或進食,必須注意喉頭反射是否已經恢復,以喝水不會嗆到為佳。但現代人普遍認為麻醉就是讓病人睡覺,輕忽了止痛的重要性。手術做完就急著離開,而忽略了麻醉後應有的休息及注意事項,值得警惕。A女的遭遇,就是診所護理人員輕忽的結果。據瞭解目前全臺灣約三成的醫師已經投入醫美市場,但是在整形的專業技術層面讓民眾充滿著問號。因此潘懷宗議員請中華民國整形外科專科醫師暨美容外科醫學會專科醫師王篤行醫師,針對上述個案做簡易說明及教導民眾有醫美需求時,需要的注意事項:. 抽脂不是用來減體重它是用來*局部雕塑*,體重太重應是用飲食、運動控制(減輕)體重後,仍有局部的肥胖才來抽脂較佳。. 抽脂需有專業有經驗的醫師執行,基本上看是否有台灣整形外科專科醫師執照或美容外科專科醫師,另外成立時間較久的醫院比較安全。而不是抽的量多就好,是要抽得均勻,所以抽脂教科書有一句名言{抽出來什麼不重要,而是留下來什麼比較重要!}. 抽完皮膚凹凸不平是抽脂不均勻的結果,抽脂的方向最好是扇狀,不要同一個部位方向抽太久,另外是要交叉才會均勻。通常能否抽脂由醫師先用手指捏起皮膚測量其厚度,如果少於一指幅就不可以抽,超過一指幅以上可以抽脂,但是抽到一指幅寬就要停止,另外抽脂剛開始抽出的脂肪是黃色慢慢變紅色,就必須要停止,以免抽過頭。. 體外超音波顧名思義是在皮膚外利用超音波軟化脂肪再用抽脂管進行抽脂,所以它比傳統抽脂較易操作,但是就容易抽過頭,因此技術上需要注意,另外超音波在體外軟化脂肪時會產生熱量需要注意不要燙傷。

. 如果造成凹凸不平,可以再找資深的醫師做修補手術。潘懷宗議員也希望臺北市政府衛生局能夠硬起來,應該針對醫美診所盡速設立相關管理辦法與查核機制,保護市民的權益。目前臺北市衛生局在99100年僅對執行「雷射」、「微整形」業務輔導項目訪查基層醫療機構,竟然就發現目前有被訪查到的基層醫療機構,居然在醫療儀器許可證明及揭示、醫師操作資格證明及揭示等規定事項合格率未達85,至於尚未被訪查到甚至不合格的診所還是未知數。而且臺北市衛生局目前只能消極的依照衛生署公告的「診所安全作業參考指引」進行督導考核,甚至連全臺北市到底有多少間合法開業的醫美診所數量都不知道,對於市面上形形色色的醫美廣告也沒有強力的管理及規範方式,讓許多市民被誇大廣告吸引後進行手術,卻達不到宣稱或預期的效果,甚至留下一輩子的傷害,實為憾事。臺北市衛生局一年接獲民眾申請醫療爭議調處就高達600件,但其實還是有多數民眾不知道有這樣的協調管道,隱藏的醫療糾紛案件數量可能更多。潘懷宗議員希望臺北市政府能多協助加強民眾醫學常識及增加醫病溝通管道,以降低醫療糾紛案件的發生,也減少社會資源的浪費。

潘議員辦公室補充之麻醉相關資料監察院在10015日通過糾正行政院衛生署乙案,糾正案文指出缺失如下:一、衛生署輕忽我國麻醉專科醫師人力不足及區域失衡之窘況,致其與麻醉護理人員位處高風險之手術環境,頻頻超時、超量及超限工作,影響國人就醫權益及醫療品質。 二、監察院於92年促請檢討改善國內麻醉醫療品質,衛生署迄未提出具體改善對策,反而指稱我國麻醉醫療人力符合我國醫療市場人力需求,有粉飾太平之嫌。 三、國內麻醉醫療業務多由麻醉護理人員協助麻醉專科醫師執行,以紓解醫師人力不足之窘況,然因麻醉護理人員資格、訓練及認證水準良莠不一,相關業務範疇及專科護理師制度亦迄未釐清及完備,其待遇又難以反應實際工作量,衛生署難辭未善盡管理及輔導職責之疏失。監察院不但對衛生署提出國內麻醉醫療品質不佳,未善盡管理及輔導糾正,還提出臺灣麻醉醫學會調查臺灣麻醉相關致死率約10萬分之17,為美國2倍、日本17倍,且國內麻醉專科醫師工作超時與超量,使人為因素引發重大麻醉併發症近4成等情,在糾正案內更提出實際受害案例:99年高雄市12歲林姓資優女童接受手腕端橈骨及尺骨骨折手術,致智商重創、97年高雄市28歲李姓產婦接受剖腹產手術,致男嬰夭折、94年新竹市王姓產婦接受無痛分娩後不治、93年台北市整型美容外科診所出現兩起接受削骨手術之女性1死、1植物人、93年陳姓4歲男童接受簡單包皮環切手術致死等事件。

morpheus贊曰: 媒體報導的似乎有些錯誤歸因及過於簡略;潘懷宗議員的部落格所言比較清楚。站長很希望能知道「麻醉過量疑慮」是從何得來?潘議員在文中雖然請麻醉專科醫師說明,可惜似乎只是隔靴搔癢。潘議員既然是藥理學教授,站長期待潘議員能以自身專業教育全國民眾(與麻醉醫師):麻醉過量引起投訴案例一(A女)症狀(特別是口齒不清╱大舌頭)的神經藥理機轉。至於「一般醫師也可以幫病患麻醉」這碼子事已經老掉牙了,不提也罷。站長不認為「泌尿科轉醫美」是原罪。將相本無種,何況在台灣現行的惡劣健保制度之下,每個醫事人員都有逃命的權利。麻醉專科醫師也有成功轉戰醫美的呀!

Child Cancer Radiation Raises Breast Cancer Risk   Risk Comparable to That Posed by BRCA GenesBy Charlene Laino WebMD Health NewsReviewed by Laura J. Martin, MDJune 4, 2012 (Chicago) -- Girls treated with radiation for childhood cancers are at elevated risk of developing breast cancer in mid-life -- even if the radiation dose was relatively low, researchers say."We were surprised to find that women treated with radiation to the chest during childhood have a risk of developing breast cancer that is comparable to that of women who are carriers of the hereditary BRCA mutations," says Chaya Moskowitz, MD, of Memorial Sloan-Kettering Cancer Center in New York City.Moskowitz and colleagues analyzed data from more than 1,200 women treated with radiation for childhood cancers and found that by age 50, 24% had been diagnosed with breast cancer. Among survivors of Hodgkin lymphoma -- who were treated with high doses of radiation that are no longer used -- the rate was 30%.By comparison, about 31% of women who carry a mutation in the BRCA1 gene, which most doctors recognize as a known risk factor, are diagnosed with the disease by age 50, she says. Among women in the general population, the rate is 4%.Previous studies have shown that women treated with radiation to the chest as children are at increased risk for breast cancer, Moskowitz says. But this is the first study to show that risk is comparable to carrying BRCA mutations, she tells WebMD.It has also not been clear how lower levels of radiation used to treat pediatric cancers other than Hodgkin's disease affect breast cancer risk, Moskowitz says.She notes that guidelines developed by the National Cancer Institute-sponsored Children's Oncology Group recommend that women treated with 20 grays -- a dosage measurement -- or more radiation to the chest begin annual mammograms and breast MRI scans at age 25, younger than recommended for average-risk women.But the study showed that women exposed to between 10 and 19 grays are also at heightened risk and therefore may benefit from early breast cancer screens, Moskowitz says.Approximately 50,000 women in the U.S. have been treated with 20 grays or higher, and an additional 7,000 to 9,000 have been exposed to 10 to 19 grays.The findings were presented here at the annual meeting of the American Society of Clinical Oncology (ASCO).12 Million Cancer Survivors"These are rather striking data," says Nicholas Vogelzang, MD, of the Comprehensive Cancer Centers of Nevada in Las Vegas and US Oncology. He was not part of the study."We have an obligation to those many thousands and thousands of young women we treated years ago. Hopefully this will increase our awareness of [the] need for mammogram screening of this population," he says.ASCO President Michael P. Link, MD, says that once active cancer treatment ends, survivors typically receive ongoing follow-up care from their primary care doctors."Patients should empower themselves and bring a summary of treatment they have received to their doctors," says Link, the Lydia J. Lee professor in pediatric cancer at Stanford University School of Medicine.He notes there are nearly 12 million cancer survivors in the U.S., up from three million in the 1970s.The work was supported by grants from the National Cancer Institute.

Print Version Sponsored By The touch-screen sensors of the Apple iPad have inspired the creation of a system that tracks the level of a physician's hand hygiene.Dr. Richard Deutsch, director of Healthquest Technologies and a retired chiropractor, has received a patent for the Safe-Hands Hygiene Monitoring System, a tool that detects whether a doctor has used a hand sanitizer or washed his or her hands before touching a patient or connecting medical equipment to a patient."The application was developed because there is currently no means of determining contact between a patient and a caregiver or health care worker," Deutsch told eWEEK.At least one-half of all hospital infections can be prevented, according to the Centers for Disease Control and Prevention. More than 100,000 people die per year from hospital-acquired infections, the CDC reported.The purpose of Safe-Hands is to determine when contact occurs between the patient and caregiver, and whether the caregiver meets the accepted standards for hygiene.Even if a doctor's hands are clean, touching a pager or cell phone before or while seeing a patient can reinfect the physician, noted Gregg Malkary, founder and managing director of Spyglass Consulting Group.In a Spyglass study, 79 percent of the doctors interviewed indicated they believed that mobile devices caused infection-control risks at the point of care."You tend to pick up your device with your gloved hands," Malkary told eWEEK. "Once the device is infected, it becomes a vector of contamination."In addition to mobile devices, centrally located keyboards, like those on hospitals' computers on wheels (COWs) workstations, are particularly likely to collect bacteria, Malkary noted.To create Safe-Hands, HealthQuest repurposed the capacitive-touch technology of the Apple iPhone and iPad to detect when a health care worker touches either the patient or medical devices such as a catheter or ventilator attached to the patient."We're taking charge-transfer technology used for detecting contact with the iPad and instead of using the iPad screen, we make the patient the screen and the health care worker the equivalent of the user's finger," Deutsch explained."When the health care worker touches the patient, which is the equivalent of the iPad screen, that generates an indication, and that is correlated with some Boolean logic of whether a hand sanitizer was used before contact with the patient or not."Sensors attached to the patient or medical equipment send signals over WiFi to an iPad-like LCD screen, which displays an animated message and voice notification informing the patient of the clinician's hygiene status. The system also uses video to record hand-hygiene violations.Although Healthquest has yet to test the prototype in an actual pilot, the company hopes to set up trials in hospitals, outpatient medical centers and long-term care facilities."No beta-testing or clinical trials have been initiated to date," said Deutsch. "[We're] still looking for the perfect strategic partner to assist in final design and commercialization prior to beta-testing."While Safe-Hands could serve a need, "innovations in this product sector have almost no clinical efficacy or trials data output that demonstrates their TCO or ROI," Shahid Shah, CEO of IT consulting firm Netspective Communications and author of the Healthcare IT Guy blog, told eWEEK in an email."This is actually one of the first tools that can be used by administration to at least get an assessment of hygiene usage among physicians, clinicians and health professionals," said Malkary. "It's not a perfect solution, but it does give you some indication of a potential problem."The use of a camera automatically taking pictures could be a turn-off, Brenda Helms, infection prevention manager for The Heart Hospital-Baylor in Plano, Texas, told FierceMobileHealthcare.Malkary sees the potential for a product such as Safe-Hands to send indicators to an electronic health record (EHR) to monitor the hygiene of a patient's caregiver.To really fight hospital-acquired infections, sensors would have to be attached to everything a doctor touches, such as door knobs, pens and paper clips, Malkary suggested.
